New Patient Information

Thank you for choosing
Mountain West Dermatology!

We know that you have a choice when it comes to selecting your dermatologist.  It is our hope that you have a positive experience at our office.  We strive to help you with all of your skin concerns.

New patients are always welcome here.  You can choose to see either Dr. Mark Hinkson, Board Ceritified Dermatologist, or our Physician Assistant, Rob Willis

If this is your first time coming to our office, we recommed that you complete the new patient packet prior to your appointment. The packet includes a map, patient information forms and medical history forms.  We will need to make copies of your insurance cards and drivers license.  Please be sure to bring them to your appointment.

We are now on electronic medical records. To reduce wait time due to data entry at your appointment, we recommend that you complete your medical history online through your secure patient portal.  Usernames and passwords are avaialble from our office staff.
